At Jai Medical Systems, our mission continues to be promoting high quality healthcare for our members.

One of the ways that we achieve high quality healthcare is through our health awareness campaign Live Life Well, which focuses on promoting healthy lifestyles and preventative care.

In addition, strategic healthcare awareness activities and special healthcare programs have been launched to encourage our members to Live Life Well.

One example is our Healthy Rewards program. As a Jai Medical Systems member, you get to earn Healthy Rewards for making healthy choices. An annual physical or well-child visit with your personal doctor (Primary Care Provider) will earn you a $25 Healthy Reward! Also, all children under 15 months of age are enrolled in the Healthy Babies Program and are eligible for Healthy Rewards for up to six (6) visits with their pediatrician.

Have you already had a visit with your Primary Care Provider and earned your Healthy Reward?  Please complete the Healthy Rewards Redemption Form to have your gift card mailed to you. You may or may not be eligible for all gift card incentives. Please contact Customer Service at 1-888-524-1999 to determine your eligibility.
